Build a scored client assessment with a PDF report
Several score areas, a results chart, personalised feedback and a downloadable report: the complete recipe.
What you'll build: a 15-25 question assessment that scores people in 3-5 areas (say Strategy, Team, Systems), then shows a chart, a paragraph of feedback per area, and a PDF they can download. One ending does all of it.
1. Plan on paper first (10 minutes)
- Pick 3-5 areas. Fewer, sharper areas beat many vague ones.
- Write 4-6 statements per area that people rate, e.g. We review our numbers every week.
- Choose one answer scale for all of them, e.g. Never · Sometimes · Often · Always = 0 to 3.
- Decide your bands per area: 0-5 Emerging, 6-11 Developing, 12+ Leading.
2. Build the questions
- Add a welcome screen that promises the resultGet your personal Growth Readiness score and report in 6 minutes. Switch on the time to complete. See Welcome screen.
- Add one Multiple Choice per statementUse the same four options each time. Group each area with a Statement intro (Part 2: Your team).
- Give questions codes
- Ask for contact details at the endBefore the ending: name and email, so you can send the report and follow up.
3. Score it

S1→ + Add points → name the column Strategy → 0, 1, 2, 3. See Points. - Copy to the rest of the areaApply to other questions… → tick

4. Personalise the result
- One write-up per areaHidden fields & variables → add a Write-up based on Strategy with Number ranges: 0, 6, 12, each with its own advice. See Write-ups.
- Build the endingTitle: Your results, @First Name. Add a Results Chart of all areas with score bands. Then each area's heading and its write-up chip. See Results chart.
- Highlight the weakest areaOptional: Pick by rank → Which one is lowest → store in
focus_area, then a highlighted personalised section: Your #1 priority: `{{focus_area}}`. - Turn on the PDFEnding → Download PDF → title Your Growth Readiness Report. See PDF reports.
5. Follow up automatically
- Email the respondent their write-ups and a link to book a call.
- Tag low scorers as Needs help and send them to your CRM.
- Show Calendly on the ending for people who'd benefit most.
6. Prove it works
